Over the Counter Medication Form

If your child needs OTC (Over the Counter)  medication during the school day for any reason, we do have stock meds on hand, however, the child must have written authorization from the pediatrician and the parent to receive medication at school. If your child will need medication over the course of a few school days, such as ibuprofen or tylenol for broken bones, please send in their own supply of medication in the bottle. 

Physical Exam Form 

All NEW STUDENTS are required to turn in a physical form from their pediatrician that is less than a year old, and an updated record of their immunizations

RETURNING STUDENTS in Kdg, 3rd, and 6th grade is required to turn in an updated physical examination form.

PHYSICAL EXAMINATION of STUDENTS (Archdiocese Policy)

The general practice in the Archdiocese of St. Louis is that students have a completed physical examination by a licened doctor of medicine (MD), doctor of osteopathy (DO), or a physician's assistant (PA), or nurse practioner (NP), working under a collaborative practice agreement with a liceneced physician upon entrance to Kindergarten, 3rd Grade, 6th Grade, and 9th Grade.  All new entrants at any grade level should have a physical examination if they have not had a physical in the past twelve months.  The physical examination must be completed and signed by a licensed MD, DO, or PA/NP working under a collaborative practice agreement with a licensed MD or DO.

The completed forms should be in the school's office by the first day of school.  The student may be excluded from school until the physical examination is completed and the form is returned.  

 

Every student is required to turn in an updated P.E. form yearly
